Alex Smith and the 49ers were still waiting Tuesday night for a determination from Dr. James Andrews on whether he will need more surgery on his injured right shoulder. Andrews, one of the top orthopedic surgeons in the country, performed Smith's shoulder surgery in December. Smith said he had shoulder pain after Friday's practice, and he was on the inactive list for Sunday's season opener, which the 49ers lost to Arizona. An MRI exam revealed a fracture, Smith said. Andrews received the test results and other information from the 49ers' team physicians Tuesday at his office in Birmingham, Ala.
